Add/Drop Withdrawal Form: classes can be dropped by the student in LoLA
Student Drop/Withdrawal Form – This form should be used when a student needs assistance with dropping or withdrawing from a course. This form should be initiated by the student.
Course Add Request Form – This form should be used when a student needs assistance with adding a course. This form should be initiated by the student.
Administrative Withdrawal Form -This form should be used when a student is requesting an administrative withdrawal from a class due to missing the published withdrawal date. This form should be initiated by the student.
Administrative Drop Delete Form – This form should be used when a student is requesting an administrative drop/delete from a class. (This form should only be used if directed by an Academic Dean or Enrollment Staff)
Course Substitution/Waiver Request – Use this form to request approval to substitute or waive a required course based on prior coursework, experience, or demonstrated competency. [Submit a Course Substitution/Waiver Request
Prior Learning Credit Request – Earn credit for college-level knowledge gained through work experience, military training, certifications, or other learning outside the classroom
Graduation Application – Ready to graduate? Complete this form to apply for graduation and ensure your eligibility for your degree or certificate.
Incomplete Grade Request Contract – Use this form to request an incomplete grade due to extenuating circumstances and outline the plan for completing remaining coursework.
Prior College Transcript Evaluation Request – Submit this form to have your previous college coursework evaluated for possible transfer credit toward your current program.
The previous school must send the records electronically or in a SEALED envelope.
Program Change Request Form – Use this form if you want to change your major or program. Be sure to get a signature from the advisor of each program, then turn it in to Student Services so we can process your request.
Request Proof of Enrollment – Use this form to obtain official verification of your current or past enrollment status for employers, insurance, or other purposes.
