Admissions Documents

After submitting an application online through our application page, located here, the items below are needed for full admission. The admissions office will communicate with students via the email address provided on the admissions application. Be sure to add [email protected] to your accepted email senders and check your account frequently. Your campus student services may also contact you directly through your campus-specific email:

REQUIRED DOCUMENTS:
  1. Immunization Records - forms available on LoLA account.
    • Measles, Mumps, Rubella (MMR) – Two doses
    • Tetanus, Diphtheria (Td/Tdap) – One dose within the last ten years
    • Meningococcal – Two doses OR one dose if first dose was given on or after age 16
  2. Proof of Registration with Selective Service - forms available on LoLA account.
    • Males between the ages of 18-25 only
Supplemental Documents:
  1. Placement Test Scores
    • ACT scores can be requested through by visiting the ACT website and entering school code: 5015.
    • Accuplacer (Math, Reading, and English) is offered it each CLTCC Campus location. 
    • Please note: Practical Nursing has REQUIRED test scores for acceptance into the program. Please visit with your campus coordinator for more information.
    • Please see Placement Testing for more information. 
  2. Transfer Credit
    • To receive transfer credit, students must submit official and final transcripts from the colleges from which the credit was received. Send transcripts to: [email protected].
    • Complete a Request for Transfer Credit to be evaluated.
  3. High School Transcript/GED/HiSET
    • While high school completion is not required for general admission, it is required for entry in to all Nursing and Allied Health programs with the exception of Nurse Assistant. High school completion is also required to be eligible to receive financial aid.
